💼 About This Role

You'll visit members in their homes to conduct in-depth health risk assessments, using your clinical expertise to evaluate medical history and identify diagnoses. Your work directly supports personalized care management by Porter's Care Guides. You'll enjoy flexible scheduling and mileage reimbursement.

🎯 What You'll Do

• Visit members in geographic area for in-home health risk assessments
• Evaluate past medical history, review of systems, and medication reconciliation
• Perform comprehensive physical exams and point-of-care testing
• Formulate list of current and past medical conditions using clinical judgment

📋 Requirements

• Valid unencumbered Michigan NP license
• 3 years of patient care experience
• Experience in primary care, adult, or geriatric settings (preferred)
