You'll provide skilled home health physical therapy to patients in Contra Costa County, helping them regain independence. You'll evaluate, develop care plans, and coordinate with physicians to ensure quality outcomes. This part-time role offers flexible scheduling and a supportive team environment.

🎯 What You'll Do

• Perform home evaluations and develop care plans
• Help patients with mobility and independence
• Collaborate with physicians and coordinate care
• Supervise Physical Therapy Assistants per regulations
• Document and report patient progress

📋 Requirements

• Current CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
