Description

You will provide 1:1 home health care visits to clients in the Lower Bucks County and Northeast Philadelphia area. You will follow care plans, administer medication, perform wound care, monitor conditions, and document care using a touch pad tablet. Requirements

• Current RN license in PA
• Minimum one year recent clinical (medical/surgical) experience
• Graduation from an accredited nursing program
• Ability to travel to cases as assigned
• Strong interpersonal and computer skills; prior EMR experience preferred

Responsibilities

• Follow designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions; report changes to Clinical or Client Services Manager
• Perform client assessments and case management and coordination
