💼 About This Role

You'll provide skilled, compassionate care in participants' homes as part of the WelbeHealth PACE program coordinating with an interdisciplinary team. You'll skip OASIS assessments and focus on patient care while earning hourly pay that includes travel time. This role offers work-life balance through flexible scheduling and built-in documentation time.

🎯 What You'll Do

• Conduct home health nursing assessments for equipment and participant goals
• Participate in IDT meetings to develop participant care plans
• Deliver nursing interventions per care plans and physician orders
• Instruct participants and caregivers on disease and self-care techniques

📋 Requirements

• Graduate of an accredited school of nursing
• Unencumbered RN License required
• Minimum one year nursing experience with frail or elderly population
• Reliable means of transportation

✨ Nice to Have

• Home health agency experience preferred
• CPR and First Aid certification upon hire preferred
