Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes, performing evaluations, developing intervention plans, and educating patients and families to promote independent living.

Requirements

• Current South Carolina Occupational Therapist license
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of functional status, including ADL and IADL performance
• Assess cognitive, psychological, and motor aspects; develop individualized intervention plans
• Evaluate home environment for hazards and identify equipment needs
• Document observations and interventions using a touch pad tablet
