Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, performing diagnostic tests, developing care plans, and educating clients and families. You will be part of a multi-disciplinary team using a touch pad tablet for documentation.

Requirements

• Current NJ Physical Therapist license
• Minimum 1 year recent work experience as Physical Therapist
• Graduation from APTA or CAAHEP approved program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, range of motion, balance, etc.)
• Implement wound prevention and repair interventions
• Develop and implement individualized care plans with manual therapy and gait training
• Educate clients and families on rehabilitative care and document using tablet
