💼 About This Role

You'll provide integrated clinical and case management to justice-involved individuals in Orange County jail facilities and post-release. You'll deliver in-person assessments and pre-release planning to improve health outcomes and reduce recidivism. This role supports successful community reintegration through care coordination and linkage to services.

🎯 What You'll Do

• Conduct clinical assessments of member needs, including medical, behavioral, and social factors.
• Develop, implement, and monitor personalized care plans in collaboration with members and providers.
• Provide in-person pre-release case management within Orange County jail facilities.
• Coordinate post-release follow-up, including wellness checks and linkage to housing and treatment.

📋 Requirements

• Ability to work in secure custodial environments and field-based settings across Orange County.
• Completion and maintenance of county jail clearance, including background checks and security protocols.
• Experience in pre-release planning and coordination of benefits for justice-involved individuals.
• Proficiency in documenting services in an Electronic Health Record (EHR) within required timelines.

• Experience with trauma-informed care and therapeutic community principles.
• Knowledge of Medi-Cal and SSI benefits enrollment.
• Bilingual in Spanish.
