💼 About This Role

You'll provide high-quality primary and preventive care to residents in skilled nursing and assisted living communities. You'll manage acute and chronic conditions while partnering closely with nursing staff to ensure exceptional patient outcomes.

🎯 What You'll Do

• Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings
• Conduct admission assessments, physical exams, medication reviews, and depression screenings
• Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care
• Counsel patients and family members on treatment options and changes in health status

📋 Requirements

• Active and unrestricted Nurse Practitioner license in Tennessee
• Current national board certification (AANP or ANCC)
• Minimum 1-2 years of experience in primary care, geriatric, or long-term care settings
