Description

You will coordinate and provide hospice care to patients and families in their homes and communities, focusing on comfort, pain management, and emotional support. Your role includes assessing patient needs, developing care plans, and collaborating with an interdisciplinary team to ensure compassionate end-of-life care.

Requirements

• Registered Nurse (RN) license
• Experience in nursing with emphasis on community health
• Ability to perform comprehensive physical assessments
• Strong communication and care coordination skills
• Willingness to participate in on-call duties

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family hospice needs
• Develop and revise plans of care based on patient status and nursing diagnosis
• Administer medications and treatments as prescribed
• Coordinate care as Case Manager and communicate with physicians and interdisciplinary team
• Educate patients and families on self-care and health management
