Description

You will make outpatient visits to senior living clients in Washington County, Vermont, performing diagnostic tests, developing care plans, and providing rehabilitative care to help clients remain at home. You'll use a touch pad tablet for documentation and work as part of a multi-disciplinary team.

Requirements

• Current Vermont Physical Therapist license
• Minimum one year recent work experience as a Physical Therapist
• Graduation from a program approved by APTA or CAAHEP
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make outpatient visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (e.g., mobility, balance, strength)
• Develop and implement individualized care plans including manual therapy, gait training, and balance interventions
• Educate clients and families on rehabilitative care for health, safety, and independent living
• Accurately document client care using a touch pad tablet
