💼 About This Role

You'll provide home health visits to seniors in senior living communities, performing admissions, assessments, and care coordination. You'll make a direct impact on patient well-being while enjoying a flexible weekend schedule.

🎯 What You'll Do

• Follow a designated care plan in accordance with patient's needs
• Perform home visits for medication, wound care, treatments, and procedures
• Monitor client conditions and report changes to Clinical Manager
• Document care using a touch pad tablet

📋 Requirements

• Current Registered Nurse license in North Carolina
• Minimum one year of recent clinical experience
• Graduation from an accredited nursing program
• Ability to travel to cases as assigned

• Prior home care experience
• Experience with electronic medical records (EMR)
• Strong interpersonal and computer skills
