Description

You will provide home health physical therapy to seniors within 1-2 communities in Carroll County, Maryland, as part of a multidisciplinary team. Your role includes performing diagnostic tests, developing care plans, and educating clients to promote health and independent living.

Requirements

• Current Maryland Physical Therapist license
• Minimum 1 year of recent work experience as a Physical Therapist
• Graduation from an APTA-accredited program
• Ability to work independently and manage time effectively
• Strong interpersonal and organizational skills; computer proficiency with EMR preferred

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, strength, balance, etc.)
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and families on rehabilitative care for health and safety
• Document care using electronic medical records on a touch pad tablet
