You will lead teams providing case management to underserved populations, ensuring high-quality care through team supervision, program operations, and staff development. You'll help redefine healthcare for communities with complex needs while collaborating with leadership and external partners.

Requirements

  • Deep understanding of Medi-Cal populations, including homelessness, mental illness, or substance use disorders.
  • 4+ years experience managing front line staff.
  • Exceptional organizational and time-management abilities.
  • Strong interpersonal and communication skills, including team coaching and conflict resolution.
  • Familiarity with compliance standards, documentation systems, and data-driven decision-making.
  • Valid driver’s license, automobile insurance, and reliable transportation (preferred).
  • Advanced training in related field (e.g., Masters in Social Work, SUD Certifications) preferred.
  • Proven leadership experience in healthcare or social services preferred.

Responsibilities

  • Ensure case managers meet required caseloads and optimize care delivery.
  • Build an engaged team and support burnout prevention.
  • Oversee patient care plans and quality standards.
  • Train and onboard new case managers.
  • Maintain a small personal caseload while prioritizing team supervision.
  • Collaborate with Director to meet audit requirements and track compliance.
  • Establish team culture of respect, empathy, and teamwork.
  • Approve timecards, time-off requests, and expense reports.
  • Conduct regular 1:1s for feedback and coaching.
  • Assign new patients to case managers.
  • Lead recruitment efforts.
  • Serve as subject matter expert in harm reduction and clinical case management.
  • Drive team towards quarterly objectives.
  • Implement quality improvement initiatives.
  • Build and maintain relationships with partner organizations.
  • Oversee patient enrollment and disenrollment.
  • Manage budget for supplies and inventory.
  • Facilitate daily huddles, weekly data reviews, and care planning meetings.
  • Contribute to monthly all-hands meetings.
  • Represent team in Health Plan Operations meetings.
  • Tailor training programs and mentor case managers.
