💼 About This Role

You'll oversee the Enhanced Care Management (ECM) program in Ventura County, managing a multidisciplinary team to coordinate care for justice-involved individuals. Your core impact is to improve health outcomes for high-need populations through community partnerships and program compliance.

🎯 What You'll Do

• Oversee ECM program development and continuous improvement in assigned county.
• Manage, supervise, and train a multidisciplinary team (Advocates, Housing Navigator, QA Specialist).
• Establish relationships with healthcare providers and community organizations.
• Ensure compliance with contract, state, and federal regulations.

📋 Requirements

• Four-year degree in Social Sciences or related field plus 3 years of administrative supervisory experience with justice-involved populations.
• Master's degree in healthcare admin, public health, social work or related field plus 2 years of administrative supervisory experience.
• Working knowledge of Medi-Cal and CalAIM initiatives.
• Valid driver's license, car insurance, and reliable vehicle.
