Description

You will provide skilled physical therapy to adult and geriatric patients in their homes, helping them maintain independence through individualized care plans. You'll be part of a multidisciplinary team and have flexible scheduling with premium pay.

Requirements

• Physical Therapist (PT) license
• Prior home health experience strongly preferred
• Ability to provide care to adult and geriatric patients
• Strong communication and documentation skills

Responsibilities

• Make visits to clients in their homes
• Perform diagnostic tests and measurements (mobility, range of motion, strength, balance)
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and families on rehabilitative care and activities
• Document observations, interventions, and evaluations using a touch pad tablet
