You'll curate provider lists for members needing care, collaborating with network providers to ensure optimal options. You'll report to the Care Navigation Team Lead. This role offers remote work within select states and a starting hourly rate of $19.00.

• Outreach providers and facilities to refine care options
• Collaborate with RNs on complex cases
• Discover and communicate trends to leadership
• Confirm and update provider data

📋 Requirements

• 1+ years of experience with care coordination in healthcare
• 1+ years of experience using Chromebook and Google Workspace
