Description

As a Home Health Physical Therapist at BAYADA, you will provide skilled rehabilitative care to adult and geriatric patients in their homes, helping them maintain independence. You'll perform diagnostic tests, develop care plans, and educate patients and families, all while enjoying a flexible schedule.

Requirements

• Physical Therapist licensure
• Prior home care experience not required
• Experience with adult and geriatric patients
• Ability to document care using a touch pad tablet

Responsibilities

• Perform diagnostic tests and measurements (mobility, range of motion, strength, balance, etc.)
• Implement wound prevention and care interventions
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Assess and revise care plans in collaboration with nursing team
• Educate clients and families on rehabilitative care and safety
