💼 About This Role

You'll provide 1:1 home health visits to clients in Central/Upper Bucks County, following a designated care plan. You'll make a direct impact on patients' well-being through medication administration, wound care, and monitoring. This role offers flexible scheduling and a supportive team environment.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to management
• Accurately document observations and interventions using a tablet

📋 Requirements

• Current Registered Nurse license in Pennsylvania
• Minimum one year of recent clinical experience
• Graduation from an accredited nursing program
• Ability to travel to client cases as assigned
