Description

You will provide medical social work services to hospice patients and their families, assessing psychosocial needs related to terminal illness. This role involves collaborating with the interdisciplinary team and ensuring high-quality, compassionate end-of-life care.

Responsibilities

• Assess psychosocial status of patients and families/caregivers related to terminal illness and environment
• Communicate findings to registered nurse and interdisciplinary group
• Provide assessment in patient's identified residence
• Assist when residence is not safe and develop alternate plan
