💼 About This Role

You'll join an interdisciplinary team to provide comprehensive care for frail seniors, ensuring they live safely in the community. You'll conduct psychosocial assessments, develop care plans, and coordinate with participants, families, and care teams. This role offers LCSW supervision hours after 6 months at no cost.

🎯 What You'll Do

• Conduct psychosocial assessments and develop care plans with IDT
• Deliver interventions including counseling, case management, and resource arrangement
• Partner with care team for smooth transitions between care settings
• Facilitate care conference meetings for coordination with participants and support network

📋 Requirements

• Master's degree in social work (MSW)
• Minimum one year of experience with frail or elderly population
• CPR and First Aid certification (or obtain within 180 days)

• Experience coordinating care conference meetings
• Experience assisting with behavioral health or substance abuse issues
