💼 About This Role

You'll provide home health visits to adult and geriatric clients as part of a multi-disciplinary care team. You'll develop individualized care plans and help clients remain independent at home.

🎯 What You'll Do

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements of mobility and strength.
• Develop and implement individualized care plans including therapeutic exercises.
• Educate clients and family members on rehabilitative care and activities.

📋 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from an APTA-approved program as indicated by transcript or diploma.
