Description

You will provide basic nursing care in a skilled nursing/rehab setting, assessing residents, implementing care plans, administering medications, and documenting accurately. You'll work closely with the interdisciplinary team to ensure high-quality care for geriatric residents.

Requirements

• Graduate from accredited school of nursing as LPN
• Current LPN license in state of employment
• IV Certification or willing to obtain within 6 months
• Current CPR/AED Certification or willing to obtain within 6 months
• Long-term care/rehab/assisted living experience preferred

Responsibilities

• Assess residents on admission and ongoing basis; document findings
• Participate in care planning and update care plans as needed
• Administer medications and treatments safely and timely
• Monitor residents for changes in condition and notify physician
• Answer call lights and assist with ADLs as needed
