🎯 What You'll Do

• Visit members and conduct in-home health risk assessments
• Evaluate past medical history, review systems, and reconcile medications
• Perform comprehensive physical exams and point-of-care testing
• Formulate list of current and past medical conditions

📋 Requirements

• Valid unencumbered Michigan NP license
• 3 years of patient care experience
• Ability to work in Kent County and 4+ surrounding counties

• Experience in primary care, adult, or geriatric
• EMR skills
