You'll conduct in-home care assessments and coordinate personal care services for seniors in a PACE program. You'll collaborate with an interdisciplinary team to ensure participants thrive at home. This hybrid role offers flexible scheduling and a supportive team environment.

🎯 What You'll Do

• Coordinate incoming calls for participant services and referrals
• Communicate schedule changes and updates with care team
• Track and monitor home care delivery and scheduling hours
• Partner with Marketing to enroll prospective participants

📋 Requirements

• Healthcare/medical licensure or equivalency
• 3+ years of professional experience
• 3+ years case management or nursing in clinical/home setting with frail/elderly

• Nursing knowledge for frail elderly and complex conditions
