💼 About This Role

You'll conduct in-home assessments and coordinate care for seniors in our PACE program, collaborating with a supportive interdisciplinary team. Your work ensures participants can thrive at home with comprehensive medical and community-based services. This role offers a hybrid schedule with remote flexibility and onsite IDT meetings.

🎯 What You'll Do

• Coordinate incoming calls about physician orders and referrals
• Communicate with participants and coordinate schedule changes
• Partner with marketing to enroll prospective participants
• Assist with staffing and scheduling activities

📋 Requirements

• Healthcare/medical licensure or equivalency
• 3 years of professional experience
• 3 years of case management or nursing experience in clinical or home setting
• Experience with frail or elderly population
