Description

You will provide skilled nursing and rehabilitative care to adult clients in their homes in Cumberland, Harnett, and Hoke counties, working as part of a multi-disciplinary health care team to help them receive medical care at home.

Requirements

• Active LPN license in North Carolina
• Minimum one year recent verifiable LPN experience
• Graduation from accredited nursing program
• Ability to travel to cases
• Strong interpersonal and computer skills

Responsibilities

• Making home visits to clients in designated territories
• Performing medication administration, wound care, treatments, and procedures
• Monitoring client conditions and reporting changes
• Following and documenting doctors' orders
• Performing client assessments and case management
