Description

You will work under the supervision of a registered nurse to provide personal care and comfort to hospice patients, observing and documenting their condition to support their end-of-life journey.

Responsibilities

• Perform personal care needs after patient assignment
• Attend to patient requests promptly
• Report inability to perform tasks to Case Manager immediately
• Provide patient and family support
