Description

You will assess residents' health status, develop and update care plans, administer medications and treatments, and communicate changes to the interdisciplinary team. This role requires critical thinking and composure in crisis situations, with a focus on delivering effective, patient-centered care.

Requirements

• Current RN license in state of practice
• CPR/AED certification or willing to obtain within 6 months
• Long-term care experience preferred
• Critical thinking and excellent communication skills
• Ability to stand/walk for long periods and lift/transfer patients up to 200+ lbs

Responsibilities

• Assess residents' health status and educational/environmental needs on admission and ongoing
• Develop and revise dynamic care plans through interdisciplinary approach
• Administer medications, treatments, and IV fluids safely and timely
• Notify physicians of changes in condition using SBARR communication
• Monitor care delivery effectiveness and address problems via QAPI process
