You'll conduct telephonic clinical assessments and care coordination for members with cancer, supporting transitions of care. You'll ensure members have access to medications, appointments, and community resources. This role combines human touch with technology to improve outcomes for thousands of people nationwide.

🎯 What You'll Do

• Conduct telephonic assessments and medication reconciliation
• Coordinate discharge plans with hospital case managers
• Monitor member progress and establish targeted support plans
• Provide referrals to PT, OT, skilled nursing, palliative care

📋 Requirements

• Active, unrestricted compact RN license required
• 5+ years of nursing experience with 3+ years oncology or case management
• Certified Case Manager (CCM) certification
• Degree in Nursing (ADN or BSN) required

• BSN preferred
• Experience with Google Suite, Slack, electronic health records
