💼 About This Role

You'll serve as a direct care and patient advocate, collaborating with the Clinical Operations team to optimize interdisciplinary care. You'll conduct telephonic outreach within 48 hours of ER discharge, perform medication reconciliation, and ensure successful transitions home. You'll also manage real-time clinical triage for incoming calls, field staff escalations, and RPM alerts.

🎯 What You'll Do

• Contact patients or caregivers within 48 hours of ED discharge for comprehensive assessment.
• Conduct medication reconciliation and provide patient/caregiver education.
• Support post-discharge order fulfillment (DME, home health care, etc.).
• Confirm follow-up provider appointment within 7-14 days of discharge.

📋 Requirements

• Unrestricted registered nursing license in state(s) of care management activities.
• Experience with real-time documentation in EHR or Care Management systems.
• Computer literacy and ability to communicate within business structure.
• Ability to assess patient symptoms and vital signs quickly and accurately.
