Description

Lead and inspire a team providing home health care to adult and geriatric clients in North Charleston, SC. You'll oversee clinical documentation, coordinate care plans, and ensure high-quality services while collaborating with office staff and representing the company in the community.

Requirements

• 2+ years RN experience in Home Health
• OASIS experience required
• 2+ years in a leadership/supervisory role
• Excellent organization and communication skills
• Strong knowledge of home health policies and procedures

Responsibilities

• Lead, inspire, and motivate field staff
• Review clinical documentation for quality and accuracy
• Assign appropriate clinicians to clients
• Develop and update individualized care plans
• Coordinate client care and act as internal case manager
