Description

You will be an integral member of a multi-disciplinary health care team providing skilled nursing and rehabilitative care to clients in their homes. You'll conduct home visits to perform clinical social evaluations, develop care plans, coordinate community resources, and advocate for clients and families.

Requirements

• Master's in Social Work (MSW) from CSWE-accredited program
• 2+ years community-based experience
• Valid driver's license and ability to travel
• Strong interpersonal and communication skills
• Computer proficiency, EMR experience preferred

Responsibilities

• Conduct home visits for clinical social evaluations and interventions
• Provide social and emotional support to clients and families
• Coordinate community resource referrals
• Participate in multidisciplinary care plan development
• Document patient care using electronic medical records
