You will provide exceptional rehabilitative care to adult and geriatric patients in their homes, performing diagnostic tests, developing care plans, and educating patients and families to promote independence. Join a multi-disciplinary team using state-of-the-art technology for documentation.

Requirements

• Experienced Physical Therapist
• Prior home health experience strongly preferred
• Valid state license
• Ability to work independently
• Excellent communication skills

Responsibilities

• Make home visits to clients
• Perform diagnostic tests and measurements (mobility, gait, balance, strength)
• Develop and implement individualized care plans including manual therapy and gait training
• Educate clients and family on rehabilitative care
• Document client care using tablet
