You'll serve as the primary care provider for whole families, delivering in-home medical, behavioral, and social care as part of a value-based startup. You'll manage a panel of families and collaborate with an interdisciplinary team to improve health outcomes.

🎯 What You'll Do

• Manage a panel of families as primary care provider
• Provide longitudinal primary, preventive, and acute care in-home and via telehealth
• Collaborate with interdisciplinary team including MDs, LCSWs, and dieticians
• Document interactions and track tasks using electronic health records

📋 Requirements

• Active, unrestricted license to practice in Louisiana
• 2+ years providing clinical services to pediatric and adult patients
• Proficiency in clinical nursing skills (venipuncture, phlebotomy, assessments)
• Full-time availability with rotating call schedule including evenings and weekends
