💼 About This Role

You'll provide rehabilitative physical therapy to home health patients, assessing mobility, gait, and transfer capabilities. As part of an interdisciplinary team, you'll develop treatment plans and instruct patients and families. This role offers person-centered care in a faith-based nonprofit setting.

🎯 What You'll Do

• Assess and evaluate patient functional status
• Provide direct physical therapy treatment
• Instruct patients and caregivers on therapeutic appliances
• Supervise Physical Therapist Assistant as needed

📋 Requirements

• Bachelor's degree in physical therapy
• Current Ohio PT license or application in process
• Valid Ohio driver's license and reliable transportation
• CPR certification required
