💼 About This Role

You'll complete OASIS Starts of Care for adult and geriatric patients in Essex County as a home care Physical Therapist. You'll provide rehabilitative care and skilled nursing to help clients remain at home. This per diem role offers great flexibility and work-life balance.

🎯 What You'll Do

• Make home visits for Start of Care admissions
• Perform diagnostic tests and measurements
• Develop and implement individualized care plans
• Educate clients and family on rehabilitative care

📋 Requirements

• Current New Jersey Physical Therapist License
• Minimum one year recent work experience as a Physical Therapist
• OASIS experience required
