Description

You will provide home health physical therapy services to patients in the Hatboro area, performing diagnostic tests, developing care plans, and educating clients and their families. This role offers flexibility and the opportunity to deliver compassionate, high-quality care in patients' homes.

Requirements

• Active and current Physical Therapist license in Pennsylvania
• Ability to travel to cases as assigned
• Excellent assessment and communication skills
• Compassionate and reliable patient care approach

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, range of motion, balance, etc.)
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and families on rehabilitative care and activities for health and safety
• Document observations, interventions, and evaluations using a touch pad tablet
