Description

You will make home visits to adult and senior clients in the east/southeast Las Vegas territory, providing skilled nursing care such as medication administration, wound care, and treatments. You'll work as part of a multi-disciplinary team to help clients remain at home, monitoring conditions and documenting care via electronic medical records.

Requirements

• Current LPN license in Nevada.
• Minimum one year of recent LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience strongly preferred.
• Ability to travel to assigned cases.

Responsibilities

• Perform home visits for medication administration, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical Manager.
• Execute and document doctors' orders accurately.
• Perform client assessments and case management.
• Document care using electronic medical records on a touch pad tablet.
