💼 About This Role

You'll provide home health social work services as part of a multi-disciplinary team, helping patients remain at home. You will conduct clinical social evaluations, identify community resources, and advocate for patients and families. This per diem role offers flexibility and the chance to make a direct impact.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Provide clinical social evaluations and plan interventions
• Identify community resource referrals for client needs
• Act as advocate for client and family to navigate community system
• Accurately document observations and interventions using touch pad tablet

📋 Requirements

• Master's Degree in Social Work
• Licensed Clinical Social Worker (LCSW or LICSW) in Massachusetts
