💼 About This Role

You'll provide palliative care to hospice clients in their homes as part of a multi-disciplinary team, affording them the opportunity to remain at home. You'll make a meaningful impact through compassionate end-of-life care.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Administer medication, wound care, treatments, and comfort care.
• Monitor clients' conditions and report changes to management.
• Follow up on and document doctors' orders.

📋 Requirements

• Current RN license in VT and NH.
• Minimum one year of recent clinical (medical/surgical) experience.
• Ability to travel to cases as assigned.

• Prior home care or hospice experience.
• Experience with electronic medical records (EMR).
