💼 About This Role

You'll join Akido's Enhanced Care Management team serving IEHP members across the Inland Empire, providing behavioral health support to individuals with serious mental illness and substance use disorders. You'll work in community and clinic settings, collaborating with an interdisciplinary team to deliver whole-person care and connect patients to community resources. This role offers the chance to make a real difference for underserved populations using harm reduction and trauma-informed approaches.

🎯 What You'll Do

• Conduct behavioral health screenings and assessments for ECM-enrolled patients
• Provide direct patient support using motivational interviewing and harm reduction
• Collaborate daily with interdisciplinary team for whole-person care coordination
• Document encounters in Care Director per CalAIM and HIPAA standards

📋 Requirements

• MSW or MFT master's degree
• 2+ years experience in behavioral health case management with complex populations
• California driver's license and reliable transportation for local travel
• Reside locally in the Inland Empire region
