💼 About This Role

You'll process and track referral requests in an EHR system, coordinating care between patients, providers, and specialists. You'll ensure timely access to specialty care for older adults in a PACE program. This role is onsite in South Los Angeles, supporting a mission-driven team.

🎯 What You'll Do

• Process referral requests from healthcare providers into the portal.
• Coordinate with specialists to schedule participant appointments.
• Serve as liaison between patients, providers, and utilization management.
• Verify and update participant records in the EHR system.

📋 Requirements

• High school diploma or GED required.
• Minimum of 2 years of experience in a healthcare setting.
• Proficiency in EHR systems and healthcare software.
• Reliable transportation to and from work locations.

• Associate's or Bachelor's degree in healthcare administration or related field.
• Medical Assistant certificate.
• Bilingual: Spanish/English.
