Description

You will plan, organize, and direct hospice care, performing comprehensive patient assessments and developing care plans to meet the needs of patients and their families. You'll work as part of an interdisciplinary team, coordinate care, and provide education and support to ensure comfort and dignity throughout the end-of-life journey.

Requirements

• Registered Nurse license
• Experience in nursing with emphasis on community health education
• Ability to perform comprehensive physical assessments and history taking
• Effective communication with physicians, patients, families, and interdisciplinary team
• Knowledge of palliative nursing and end-of-life care

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments
• Develop and revise individualized care plans based on nursing diagnosis and patient needs
• Administer medications and treatments as prescribed
• Act as Case Manager, coordinating patient care for assigned caseload
• Participate in interdisciplinary group meetings and on-call duties
