💼 About This Role

You'll provide home health occupational therapy visits for adult and geriatric patients in Monmouth County. You'll be integral to a multi-disciplinary team delivering rehabilitative care that helps clients remain at home. This role offers a $10,000 sign-on bonus for full-time candidates.

🎯 What You'll Do

• Perform thorough evaluations of patients' functional status including ADLs and IADLs.
• Develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environment for hazards and recommend equipment adaptations.
• Educate patients and families on rehabilitative care and activities.
• Accurately document observations and interventions using a touch pad tablet.

📋 Requirements

• Current New Jersey Occupational Therapist license.
• Minimum one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.

• Prior home care experience preferred.
• Experience with electronic medical records (EMR) preferred.
