You will deliver personalized, high-quality primary care to seniors in their homes and communities, integrating primary, psych, palliative, and home-based services while building deep expertise in dementia-informed care.

Requirements

• Licensed Physician Assistant or Nurse Practitioner
• Heart for senior care and passion for redefining health for older adults
• Ability to work autonomously with team-based collaboration
• Experience or willingness to develop dementia-informed care expertise
• Commitment to compassion, consistency, and clinical excellence

Responsibilities

• Deliver personalized high-quality care to seniors in homes, assisted living, hospice, and nursing facilities
• Provide dementia-informed, relationship-based care across the continuum of care
• Collaborate with multidisciplinary team in palliative, psych, and neurology
• Support caregiver and patient outcomes through structured onboarding and coaching
• Maintain encounter volume and patient retention goals
