💼 About This Role

You'll provide hands-on therapeutic interventions in patients' homes, building core clinical skills with strong mentorship support. You'll assist with gait training, balance exercises, and home safety while collaborating with a Physical Therapist Case Manager. 🎯 What You'll Do

• Provide therapeutic interventions like gait training and manual therapy.
• Conduct patient evaluations and implement care plans.
• Document patient care accurately within 24-48 hours.
• Educate patients and caregivers on exercises and safety techniques.

📋 Requirements

• Current California PT License.
• Current BLS/CPR Certification.
• Valid driver's license with clean record.
• Graduate of an accredited Physical Therapy program.
