You'll lead and mentor a team of Health Coaches, driving patient outcomes for Medicaid populations in a hybrid setting. You'll shape team culture and performance through coaching and feedback. This role combines people management with direct impact on health equity.

🎯 What You'll Do

• Manage people and performance of assigned Health Coaches
• Lead weekly team meetings to discuss successes and challenges
• Monitor performance dashboards and provide ongoing feedback
• Shadow Health Coaches and coach on patient engagement skills

📋 Requirements

• 3+ years of supervisory experience
• Demonstrated success in managing people and guiding performance
• Experience navigating social determinants of health needs
• Strong customer service or patient-facing experience

• Experience utilizing Motivational Interviewing
• Experience working with remote teams and promoting team culture
