💼 About This Role

You'll provide comprehensive medical care to residents in skilled nursing and senior living settings, managing complex conditions with clinical autonomy. You'll build strong clinical relationships while supported by an interdisciplinary team, reducing hospitalizations and improving outcomes. This role serves vulnerable populations with a focus on quality over volume.

🎯 What You'll Do

• Provide primary, urgent, and preventive care to residents.
• Independently evaluate, diagnose, and treat acute and chronic conditions.
• Conduct initial admissions and follow-up assessments.
• Collaborate with facility Medical Directors and interdisciplinary teams.

📋 Requirements

• Active, unrestricted Nurse Practitioner license in applicable state(s).
• Current national NP certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.

• Experience in internal medicine, geriatrics, cardiology, or post-acute care.
• Active DEA registration or ability to obtain.
• Prior experience in SNF, LTC, ICU, or Emergency Department settings.
