Description

As a home care Physical Therapist, you will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, helping them receive the medical care required to remain at home. You will be an integral member of a multi-disciplinary health care team.

Requirements

• Current North Carolina Physical Therapist license
• Minimum one year of recent work experience as a Physical Therapist
• Graduation from a program approved by the American Physical Therapy Association or Committee on Allied Health Education and Accreditation
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem solving skills

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, range of joints, transfer status, stability, ambulation, strength, balance, safety)
• Develop and implement individualized care plans including manual therapeutic exercises, gait training, and balance interventions
• Educate clients and family members on rehabilitative care and activities to promote health, safety, and independent living
• Document observations, interventions, and evaluations using a touch pad tablet
