Description

You will provide home health visits to adult and geriatric clients, performing diagnostic tests, developing care plans, and educating clients and families to promote health and independent living.

Requirements

• Current New Hampshire Physical Therapist license
• Minimum 1 year recent experience as a Physical Therapist
• Graduation from an APTA or CAAHEP approved program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, balance, strength, etc.)
• Implement wound prevention and treatment interventions
• Develop and adjust individualized care plans
• Educate clients and family members on rehabilitative care
